Provenance for Every Answer

The difference between an AI answer you can act on and one you can't is provenance: the ability to see what the answer was built from. Why "sounds right" isn't good enough

'Demonstrate the answer's sources' is provenance. Recording it by default means these obligations are satisfied by construction rather than reconstructed under pressure.
